Asian Eyelid Surgery

Creating the double eyelid is the most requested cosmetic procedure in the Asian population both in Asia and the United States. The common misperception of Asian people seeking eyelid surgery is that they want to look more western. In fact the vast majority of such patients simply want a more attractive eyelid appearance while still maintaining their Asian identity. Dr. Ahn feels that this concept is extremely important in creating the eyelid crease in Asian patients.

Highlights of the Surgery:
You can expect the position of the crease to change during the healing process. In the beginning it will look too high, but as the swelling subsides and the tissues relax, the crease will lower to its anticipated height. This healing process may take several months to occur though it is largely stable by 4-6 weeks. Again, every effort will by made by Dr. Ahn to achieve the exact result desired. Often, however, there is slight asymmetry between the height of the eyelids that cannot be avoided and is part of the inexact healing process.

In fact, 50% of Asian people are born with a natural eyelid crease. The Asian eyelid crease, however, tends to differ from that of the Occidental type in being closer to the eyelashes and having a slightly different shape. The absence of an eyelid crease results from anatomical differences between the Asian and Western eyelid. This is not to say, however, that all Asian eyelids look the same. In fact, there are often differences between the Asian ethnic groups (Korean, Japanese, Chinese, Vietnamese, etc.) and with children of mixed races.

The attractive upper eyelid has little excess skin and fat, has a well-defined crease (making the eyes look more open), and has some skin showing between the crease and the lashes. This makes it easier to apply makeup for women. The attractive lower eyelid has no puffiness and smooth skin.

During your consultation with Dr. Ahn, most of the time will be spent on determining the shape and height of the eyelid crease you are seeking. The shape of the eyelid can be oval (tapered crease) or round (parallel crease). The most common choice is the oval shape resulting from the tapered crease as it seems to be the most natural for the Asian eye. The height of the crease refers to the distance from the eyelashes and is divided into small, medium and large. The height of the Asian eyelid is typically 2-5 mm from the eyelashes while that of the Western eyelid is 7-11 mm.

Every effort will be made to achieve the desired shape and height, though perfection cannot always be achieved, even in the hands of an expert. In addition, patients often bring in pictures of models in magazines as a goal for surgery. Achieving this look is often not possible.

Though other techniques using sutures for creating a crease have been reported, Dr. Ahn feels that the skin incision technique is much more precise, reliable and long-lasting. During your consultation you will also discuss whether the epicanthal fold will need to be addressed. This is the web of skin between the side of the nose and inner corner of the eye that is present in many Asian eyelids. Often, this does not need to be addressed, depending on the wishes of the patient.

He will educate you about the surgery and address any questions or concerns that you may have so that you can make a well-informed decision. Much of the consultation time will be spent determining the desired shape and height of the eyelid crease, along with whether or not the epicanthal fold needs to be addressed. The potential results will be demonstrated using digital photography and computer imaging.

Q-What sort of anesthesia is Asian eyelid surgery performed with?
